How to Register an Adverse Event Following Immunisation?

Process for registering AEFI

It is the duty of every Indian that any citizen dies after taking the vaccine or if there is any kind of complaint, then this information must be given on the Government of India’s website AEFI. Get Adverse Effects registered on the site. So that the Government of India and their doctors and scientists can get every information about the vaccine. So that if there is a need for further improvement in the vaccine, then it can be done.
You may go through the following steps to file your AEFI:

First go to the Vaccination Center where the victim was administered the previous dose.
At the vaccination center, try to locate the medical officer (MO) in charge and ask him to help you register the AEFI.
In case the MO refuses, ask for the contact number of the District Immunization Officer. You may try to register with the DIO.
In case the DIO refuses, you may locate the Civil Surgeon or District Medical Office, and try with him.
The DMO reports to the District Collector, and he may be approached as the final authority at District Level.
You may use the draft letter presented below to register your AEFI Complaint.

AEFI दर्ज करने की प्रक्रिया

हरेक भारतीय का कर्तब्य बनता है कि किसी भी नागरिक की मौत वैक्सीन के लेने के बाद होती है या किसी भी प्रकार की शिकायत हो तो ,इस बात की सूचना जरूर से भारत सरकार के वेब साइट AEFI. Adverse effects following immunization पे दर्ज करवाये। ताकि भारत सरकार को तथा उनके डॉक्टरों व बैज्ञानिकों को वैक्सीन से होने वाली हरेक जानकारी मिल सके। जिससे आगे वैक्सीन में सुधार की आवश्यकता हो तो की जा सके।

पहले वैक्सीन सेन्टर पे जाना है।
वही से प्रभारी चिकित्सा पदाधिकारी का नंबर मांग के बात करे कि मुझे अपना केस AEFI में दर्ज कराना है । अगर अनांकनी करे तो DIO ( डिस्ट्रिक्ट इम्यूनाइजेशन अफसर ) का नंबर मांग के उससे बात करे । वो मान जाएँगे । न माने तो सिविल सर्जेन से बात करे। परंतु AEIF में दर्ज कराना अनिवार्य है ।

नीचे भेजे गए ड्राफ़्ट पत्र का उपयोग करे और ऊपर बताए गए लोगों को AEFI दर्ज करने के लिए सम्पर्क करे।
